FEES AND POLICIES
Please call me at 626-792-0773 or click on the email link to email me a question and I will respond to you within
24 hours. My fee is $125.00 - $140.00 for a 50 minute session.
Payment in the form of check, cash or credit card (VISA and Master card) is due at the time of the session unless
you and I have made previous arrangements.
INSURANCE BILLING
I currently do not accept insurance. If you have a PPO insurance plan and your plan allows "Out of Network
Providers" I can provide you with a statement to submit to your insurance.
Advantages when you pay out of pocket include:

I have very flexible hours Monday through Thursday ( 9:00 am - 9:00 pm) and can accommodate most requests.
Please provide 24 hour notice when you must cancel your appointment. When you and I select a time for your
appointment I set that time aside for you and am unable to fill it without prior notification. I will bill you for a missed
appointment when you fail to provide timely notice.

- You and your therapist decide the length of your treatment without insurance policy limits.
